| The Internet address provided by the shipping company where shipments can be tracked online. | |||
| Tracking Ticket Number | |||
| The tracking number printed on your tracking receipt supplied by the shipping company. | |||
| Shipping Status | |||
| Enter here the status of the shipment as of the last time you checked before sending the form. | |||
| Did you contact the customer after shipping the product and supplied a tracking number? | |||
| Enter Yes or No for the answer that applies. | |||
| If you entered No, explain | |||
| Explain the reason for not contacting the customer with the tracking information. | |||
